I\u2019m excited to join you and support your student\u2019s health and wellness throughout the year. As the school nurse, my goal is to ensure your student has a safe and healthy environment to learn and thrive in.<\/p>\n\n\n\n<p>To help us prepare for a smooth start, please review the following important health-related reminders and requirements:<\/p>\n\n\n\n<p>\ud83d\udd04<strong>Annual Health Forms \u2013 Required Each School Year<\/strong><\/p>\n\n\n\n<p>If your student has any of the following conditions, please make sure the appropriate form is completed and returned to the nurse\u2019s office <strong>each year Prior to 1<sup>st<\/sup> day of school<\/strong>:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Seizure Action Plan<\/strong><\/li>\n\n\n\n<li><strong>Asthma Action Plan<\/strong><\/li>\n\n\n\n<li><strong>Allergy\/Anaphylaxis Plan<\/strong><\/li>\n\n\n\n<li><strong>Diabetes Management Plan<\/strong><\/li>\n\n\n\n<li><strong>Religious Exemption Form<\/strong> (must be notarized)<\/li>\n<\/ul>\n\n\n\n<p><strong>Forms can be accessed through the link provided below or you may stop by for a copy:<\/strong><\/p>\n\n\n\n<figure class=\"wp-block-embed is-type-wp-embed is-provider-kenai-peninsula-borough-school-district wp-block-embed-kenai-peninsula-borough-school-district\"><div class=\"wp-block-embed__wrapper\">\n<blockquote class=\"wp-embedded-content\" data-secret=\"4rsihChWnX\"><a href=\"https:\/\/kpbsd.org\/departments\/assistant-superintendent\/instructional-services\/health-services\/\">Health Services<\/a><\/blockquote><iframe loading=\"lazy\" class=\"wp-embedded-content\" sandbox=\"allow-scripts\" security=\"restricted\" style=\"position: absolute; visibility: hidden;\" title=\"&#8220;Health Services&#8221; &#8212; Kenai Peninsula Borough School District\" src=\"https:\/\/kpbsd.org\/departments\/assistant-superintendent\/instructional-services\/health-services\/embed\/#?secret=iyU9Xn1NeV#?secret=4rsihChWnX\" data-secret=\"4rsihChWnX\" width=\"600\" height=\"338\" frameborder=\"0\" marginwidth=\"0\" marginheight=\"0\" scrolling=\"no\"><\/iframe>\n<\/div><\/figure>\n\n\n\n<p><strong>\ud83d\udc8a<\/strong><strong> Medication at School<\/strong><\/p>\n\n\n\n<p>If your student needs to take <strong>daily medication<\/strong> at school:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The medication must be brought in <strong>by a parent\/guardian<\/strong>.<\/li>\n\n\n\n<li>It must be in the <strong>original pharmacy-labeled container<\/strong> and <strong>not expired<\/strong>.<\/li>\n\n\n\n<li>A <strong>Medication Authorization Form<\/strong> must be signed <strong>by both the parent\/guardian and the healthcare provider<\/strong> <em>before<\/em> any medication can be administered at school.<\/li>\n<\/ul>\n\n\n\n<p><strong>\ud83d\udc89<\/strong><strong> Immunizations<\/strong><\/p>\n\n\n\n<p>If your student received <strong>any immunizations over the summer<\/strong>, please be sure to submit updated records to the school <strong>prior to the first day of school<\/strong>.<\/p>\n\n\n\n<p><strong>\ud83e\ude7a<\/strong><strong> Physical Exams \u2013 Required for New Students<\/strong><\/p>\n\n\n\n<p>If your student is <strong>new to the district<\/strong>, a physical exam is required before attending classes. Please provide documentation to the school nurse as soon as possible.<\/p>\n\n\n\n<p>If you have any questions, need help locating a form, or would like to speak with me directly, feel free to contact the nurse\u2019s office.
