This will open a new window with directions.<\/li><li> From this page click on &#8220;PowerSchool Forms Portal, Begin Here&#8221;. This page also has helpful tips about completing forms, as well as, a video with instuctions.<\/li><li>If you are new to KPBSD and haven&#8217;t ever created a PowerSchool account, then click on &#8220;Register New Account&#8221;. Otherwise, log in using your PowerSchool Forms Portal username\/password.<\/li><li>Once you are logged in, you will have access to the forms. For new students, you will need to &#8220;add student&#8221; and then complete the new student enrollment forms. <\/li><\/ol>\n\n\n\n<p class=\"has-text-align-left\">In addition to a completed registration packet, we will need: <\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Birth Certificate<\/li><li>Immunization records (or a completed VacTrak form)<\/li><li>School physical: completed within 90 days of the first day of school<\/li><\/ul>\n\n\n\n<p>*If your child attended Paul Banks last school year, you do not need to re-register them, instead, please notify the office.
