Dear Kenai Middle School Families,
Welcome to a new school year! I’m excited to join you and support your student’s health and wellness throughout the year. As the school nurse, my goal is to ensure your student has a safe and healthy environment to learn and thrive in.
To help us prepare for a smooth start, please review the following important health-related reminders and requirements:
🔄Annual Health Forms – Required Each School Year
If your student has any of the following conditions, please make sure the appropriate form is completed and returned to the nurse’s office each year Prior to 1st day of school:
- Seizure Action Plan
- Asthma Action Plan
- Allergy/Anaphylaxis Plan
- Diabetes Management Plan
- Religious Exemption Form (must be notarized)

💊 Medication at School
If your student needs to take daily medication at school:
- The medication must be brought in by a parent/guardian.
- It must be in the original pharmacy-labeled container and not expired.
- A Medication Authorization Form must be signed by both the parent/guardian and the healthcare provider before any medication can be administered at school.
💉 Immunizations
If your student received any immunizations over the summer, please be sure to submit updated records to the school prior to the first day of school.
🩺 Physical Exams – Required for New Students
If your student is new to the district, a physical exam is required before attending classes. Please provide documentation to the school nurse as soon as possible.
