NEW STUDENT & KINDERGARTEN REGISTRATION
Kindergarten: All students entering kindergarten for the 2023-24 year must turn 5 by September 1, 2023.
Registration packets are now available online! Please click the link below. Complete all the required forms and sign them digitally. Forms will be sent directly to Chapman School.
Tips for using the online Registration Form:
- make sure you select Chapman School from the list of schools
- make sure you fill out all required boxes – if you do not it will not allow you to sign/submit
- if something does not apply to you but is a required field type in N/A
In addition to these forms, we will need a copy of your child’s Birth Certificate, current Immunization Records. All students new to the district must have a school entry physical. Physical Form: Physical Form (must be completed by a physician).
